Progress in islet transplantation depends entirely on complete, high-quality medical data, including the information you consented to report to the Collaborative Islet Transplant Registry (www.citregistry.org). Your long-term follow-up since your islet transplant is vitally important. To make it as easy as possible for you to provide updated information about your health, you can complete the following questionnaire on-line or mail it to your transplant center. This information is very important in the continuing search for a cure for Type 1 diabetes.
